Our office accepts the following insurance plans: We accept most major insurance plans. Ask about yours. Care Credit financing is available. For more information about CareCredit, please click on the logo below.

Please call our office to see if your insurance is accepted. Patients using insurance are expected to pay co-payments and/or deductibles at the time of service.

Payments or financial arrangements for routine services such as examinations, teeth cleaning, x-rays and emergency visits are expected at the time services are provided. Insurance claims can be filed by the office or you may file your own claim and be reimbursed directly. A written estimate will be provided for all complex treatments. We gladly give you a written estimate for any routine procedures upon request. If you have insurance coverage, a pre-determination can be sent and financial arrangements for the balance not covered by insurance can be made when this information is received.

A down payment is required for more involved, complex or extended treatment, regardless of insurance coverage or pre-determination of payment by your insurance company.

We strongly believe that treatment should be based on your needs, not the needs of insurance companies. Therefore, we do not accept direct assignment. Please be aware of the fact that your insurance coverage is a contract between yourself, your employer and your insurance company, not our office. As a courtesy, we will file your claim electronically and answer as many questions as possible regarding your coverage, but inevitably any reimbursement disputes must be directed directly to your employer or your insurance company.
